Place the performer at center stage with the vocal mic downstage and the guitar DI beside the performer.
Place one wedge in front, angled away from the vocal microphone to reduce feedback.

Equipment notes

State whether the guitar output comes from the pickup, a preamp, or a pedalboard.

Write whether the performer needs more vocal, more guitar, or a balanced mix.
If in-ear monitors are used, note that the house wedge is not needed.

Forgetting the vocal and guitar balance needed in the monitor.
Leaving the power need for a tuner, preamp, or pedalboard out of the stage plot.
